This would be for 12,000 subjects, probably an interviewer administered questionnaire.  Unless pre-testing suggests individuals can follow all the branches and triggers.  I'm not wedded to BLAISE, but the AHQR specifically requested a review of this product and competitors. Personally, I'd prefer to build a new application around the survey..but that's me.  :)
