It was intended constructively and collegially, and seemed appropriately placed: This list is a great locale for discussing online survey design, including the impact of variations on response rates, and for explaining the information being gathered for this survey and, more generally, for validity and privacy issues in online research.

Poor form to start w/ personal information and demographics. Surveys should start with topical questions to build rapport. Initials, age, email address, all right up front, are certain to make response rates nosedive.

Can you say why initials and email address are needed? There's no explanation up front, other than that you provide a guarantee of confidentiality (rather than anonymity).

Please consider the following information before deciding to participate in this research.

PURPOSE: To study the contexts in which swear words/phrases are used.

WHO IS CONDUCTING THIS STUDY: This study is being conducted by Merin Sanil, Research Intern at the Department of Psychology, Monk Prayogshala ( mes at monkprayogshala.in), under the supervision of Hansika Kapoor, Research Author at Monk Prayogshala (hk at monkprayogshala.in).

HAS THIS STUDY BEEN APPROVED? Yes, this study has received Ethical Approval from the IRB at Monk Prayogshala, in November 2013 (#013-001). For queries regarding the same, you may contact aa at monkprayogshala.in

WHAT YOU WILL DO: You will first begin by providing some basic information about yourself. Following this you will be presented with a few statements and asked to select how likely you would use the swear words/phrases in a similar situation. The task will take about 15 minutes to complete. There are no right or wrong answers to any of the items and this test is in no way a test of your intelligence. Please respond honestly.

RISKS: Your participation in this study is completely voluntary and there are NO anticipated risks with participating. Please note however that the study contains the use of profanity and abusive language and you can choose to withdraw at any given point during the study if you begin to feel uncomfortable. To do so, you may exit the study by closing your browser window.

BENEFITS: On completion, your email id will be entered into a raffle where you can win a Rs. 500/- Flipkart voucher! :) PLEASE NOTE THE MORE PEOPLE YOU GET TO FILL IN THIS FORM, THE BETTER CHANCE YOU HAVE OF WINNING!
